Your dog has mild/moderate subluxation on right side and severe subluxation on left side. For sure she has CHD but I suspect the left side might enhance by trauma. Did your dog got hurt recently or before? The TPO surgery will help the right side (conservative management also can maintain it ), for the left side I can't give you exact opionion before I can review the x-ray directly( I need to see detail condition of bone structure)
Please start conservative management now and have your dog recheck by your vet. in about 4-6 weeks later. If the condition is getting worse, surgery might be necessary.
