Yes your dog has severe CHD with almost complete luxation on both hip joints. I don't think the improving rate of TPO on your dog is very good because the acetabulum is shallowing already. Please try to follow the conservative management and I still think your dog still can walk OK after rear leg muscle is strong enough. Swimming is the best way to enhance the muscular strength.

You can start to keep her on high dose chonroprotective agent(personal I recommend Cosequin which is the best one in market as I know) for about 4-6 weeks, then reduce to maintenance dose.
If your dog is in pain now, please keep her on pain control medicine for 7-10 days(make sure liver and kidney are OK and protect stomach as needed).
You can start swimming as soon as possible once the pain is controlled. Please try to increase the swimming time gradually. Be careful the in and out of the swimming pool area to prevent any traumatic damage.
You can buy a book of "Yellow's Steps" from this website. The book has complete information to tell you how to take care of your CHD dog. Really has to say thanks to Amy for writing this book.
