Sheltie face skin problem

I have a Sheltie boy 8 months old, his face always become red and scratch and then hair off and the skin will become red and a little bit up, then I use some cream and it become pink colour after a few hours and then after about 2 to 3 weeks, the hair will grow and become normal but after a few weeks, it become the same thing again.

Is there any method that I can treat him so that this will not happen again?

Could you give me some pictures of the skin lesion and let me know what kind of cream you were using?
